Community LIFE is part of the nationally recognized Program of All-Inclusive Care for the Elderly (PACE), a model designed to help older adults remain safely in their homes and communities. Community LIFE follows a coordinated approach that integrates medical care, rehabilitation, social services, home support, transportation, meals, and preventive care into one seamless program.
For over 25 years, Community LIFE has been providing services to seniors age 55 and older who can continue living at home with the right supports. According to the National PACE Association, participants rely on help with essential daily activities such as bathing, dressing, eating, toileting, walking, and transferring. Many also manage multiple chronic conditions, including dementia, diabetes, heart failure, COPD, vascular disease, and various mood disorders.
At Community LIFE, participants receive services from an interdisciplinary team that includes primary care providers, registered nurses, therapists, dietitians, social workers, home care coordinators, personal care attendants, and transportation staff. This coordinated, teambased approach ensures that each person’s medical, emotional, and functional needs are addressed together rather than through fragmented systems of care.
The effectiveness of the PACE model is reflected in strong health outcomes. According to the National PACE Association, participants experience a 24 percent lower hospitalization rate than comparable dually eligible individuals receiving Medicaid nursing home services, and rehospitalizations are 16 percent lower than the national rate for older adult dual beneficiaries.
Preventive care is also stronger within PACE, with more consistent access to hearing and vision screenings and vaccinations, which supports longterm wellbeing. Participant satisfaction is high, averaging 4.1 out of 5, and caregiver satisfaction is equally strong, with over 95 percent saying they would recommend their PACE program to others.
